INSTRUCTIONS

(1) Subjective Assessment: You must formulate a differential diagnosis before using the calculator. Only score +3 for "PE is #1 diagnosis" if, based on history and physical exam, PE is truly the most likely cause of the patient's symptoms. (2) Malignancy Definition: Score +1 only if the patient has active cancer, meaning they are currently receiving treatment, received treatment in the last 6 months, or are receiving palliative care. (3) Immobilization: This is defined as strict bed rest for at least 3 consecutive days (bathroom privileges only).

Clinical signs and symptoms of DVT*

PE is #1 diagnosis OR equally likely*

Heart rate >100*

Immobilization at least 3 days OR surgery in the previous 4 weeks*

Previous, objectively diagnosed PE or DVT*

Hemoptysis*

Malignancy w/ treatment within 6 months or palliative*
