2012 ACR/EULAR Classification Criteria for Polymyalgia Rheumatica (PMR)

A clinical and ultrasound-supported scoring system for classifying polymyalgia rheumatica in patients with suggestive symptoms.

Age 50 years or older*

Bilateral shoulder aching/pain*

Abnormal CRP and/or ESR*

Symptoms are better explained by an alternative diagnosis*

Morning stiffness duration*

≤45 min0

>45 min+2

Hip pain or limited range of motion*

RF or ACPA*

Other joint pain*

At least one shoulder with subdeltoid bursitis and/or biceps tenosynovitis and/or glenohumeral synovitis (either posterior or axillary) and at least one hip with synovitis and/or trochanteric bursitis*

Both shoulders with subdeltoid bursitis, biceps tenosynovitis or glenohumeral synovitis*
