INSTRUCTIONS
Enter all variables using values obtained within the first hour of PICU admission. For systolic blood pressure, record the first measured value; if the patient is in cardiac arrest, enter 0, and if the patient is in shock or systolic blood pressure cannot be measured, enter 30; if no value is available and none of these apply, use the default value of 120 mmHg. Enter the first base excess value (positive or negative), noting that the calculator uses the absolute value; if unavailable, leave blank, and it will default to 0.
For oxygenation, enter FiO₂ and PaO₂ measured at the same time; FiO₂ may be entered as a fraction or percentage depending on the selected unit; if either value is unavailable, leave both blank, and a normal default will be applied. Indicate whether mechanical ventilation occurred at any point during the first hour of PICU admission. Select “elective admission” only if the ICU admission was planned (e.g., routine postoperative monitoring) rather than due to acute deterioration.
For pupillary findings, select “yes” only if both pupils are fixed to light and >3 mm, and do not include cases explained by drugs, toxins, or local ocular pathology. For the reason for ICU admission, select a recovery option only if the primary reason for admission is postoperative or post-procedural care; otherwise, select that admission is not primarily for recovery. For diagnosis risk categories (low-, high-, and very high-risk), select an option only if that diagnosis is the main reason for ICU admission; otherwise, select “none.”
Ensure all selections reflect the primary clinical reason for ICU admission rather than comorbidities, and interpret results in the context of overall clinical assessment.
