Select all present clinical risk factors for the patient: tumor size ≥3, multifocal disease, early recurrence (<12 months), frequent recurrence (>1), and failure of prior intravesical treatment. The calculator totals the risk factors to categorize the patient into low, intermediate, or high intermediate-risk tiers.
